By signing this paper you are acknowledging that you understand that the proper operation of a firearms training class depends on the careful control of a deadly weapon and such control depends on me and my classmates' wholehearted cooperation of the F.T.A. safe gun handling practices. Further, I understand that my participation in the clinic may be terminated at any time during the course if my cooperation is not deemed satisfactory by the F.T.A. representative. I agree to abide meticulously by any and all safety procedures required. I further agree to sign a statement releasing the F.T.A. and it's official representatives from responsibility for any injury that I may sustain during the training program. In signing this application, I certify that I will be at least eighteen years of age at the time of the class or will be sixteen and accompanied by my parent or legal guardian. I am aware, acknowledge and authorize F.T.A. to deduct the above amount from my credit card plus a two percent surcharge.
